Improved Prediction of Sudden Cardiac Death Risk

چکیده

Risk stratification of patients to identify those who will benefit most from the primary prevention implantable cardioverter-defibrillator (ICD) remains a challenge. For the last decade, measurement of the left ventricular ejection fraction (LVEF) as a clinical risk predictor is recognized to be inadequate. In fact, contemporary real world data indicate that of the patients identified using this methodology, the proportion of patients who benefit from the ICD at the end of a year may be as low as 1.1%. It is also accepted, however, that novel noninvasive imaging techniques will make major strides toward this goal in the near future. In this context, the low-hanging fruit is the potential role of risk markers measured from the same echocardiogram that would provide the LVEF. In this issue of Circulation: Cardiovascular Imaging, Konety et al report their results regarding the association between multiple echocardiographic predictors and risk of sudden cardiac death (SCD) in a meta-analysis performed from the ARIC (Atherosclerosis Risk in Communities) and CHS (Cardiovascular Health Study) cohorts. It is worth deliberating on their findings in the setting of existing knowledge, as well as the specific methodology used in their analysis.

Apical Hypertrophic Cardiomyopathy in a Case with Chest Pain and Family History of Sudden Cardiac Death: A Case Report

Hypertrophic cardiomyopathy (HCM) is the most common genetic cardiovascular disease, which is caused by a multitude of mutations in genes encoding proteins of the cardiac sarcomere (1). Apical hypertrophic cardiomyopathy (AHCM) is an uncommon type of HCM. The sudden cardiac death is less likely to occur in the patients inflicted with AHCM (2). Herein, we presented the case of a 29-year-old man ...

Left Ventricular Mass and the Risk of Sudden Cardiac Death: A Population‐Based Study

BACKGROUND Left ventricular (LV) mass ascertained using echocardiography may enhance risk stratification for sudden cardiac death. The objective of this study was to assess the association between left ventricular mass and the risk of sudden cardiac death in a population-based cohort and determine its incremental value beyond conventional risk predictors. METHODS AND RESULTS Assessment of LV ...

Alternative research funding to improve clinical outcomes: model of prediction and prevention of sudden cardiac death.

Although identification and management of cardiovascular risk markers have provided important population risk insights and public health benefits, individual risk prediction remains challenging. Using sudden cardiac death risk as a base case, the complex epidemiology of sudden cardiac death risk and the substantial new funding required to study individual risk are explored. Complex epidemiology...

Near-term prediction of sudden cardiac death in older hemodialysis patients using electronic health records.

BACKGROUND AND OBJECTIVES Sudden cardiac death is the most common cause of death among individuals undergoing hemodialysis. The epidemiology of sudden cardiac death has been well studied, and efforts are shifting to risk assessment. This study aimed to test whether assessment of acute changes during hemodialysis that are captured in electronic health records improved risk assessment. DESIGN, ...

Risk factor and prediction modeling for sudden cardiac death in women with coronary artery disease.

BACKGROUND To our knowledge, the risk of sudden cardiac death (SCD) and the assessment of risk factors in prediction models have not been evaluated in women with coronary artery disease (CAD). We sought to evaluate the incidence of SCD as well as its risk factors and their predictive accuracy among a population of women with CAD. METHODS The Heart and Estrogen/progestin Replacement Study eval...

مرگ ناگهانی قلبی در ورزشکاران و راهبردهای پیشگیری از آن: مقاله مروری

Sudden cardiac death in sport, although rare, but is a tragic event, attracting the media and public attention. Sport and exercise may act as a trigger for sudden cardiac death. Risk of sudden death in young athletes with cardiovascular disease is 2.5 times more frequent than non-athlete individuals. More than 90% of cases of sudden death occur during or immediately after training or competitio...
